The National Child Assessment Form is a checklist of skills and behaviors a child is likely to develop from birth to 12 months of age. Each item in the checklist is a sample of many related skills and behaviors and in that sense serves an an index of more general characteristics of development. Items in the Assessment Form are grouped into five scales that represent areas of child development: Social-Emotional, Language, Cognitive, Gross Motor, and Fine Motor.
