The National Child Assessment Form is a checklist of skills and behaviors a child three to six years of age is likely to develop. Each item in the checklist is a sample of many related skills and behaviors and in that sense serves as an index of more general characteristics of development. Items in the Assessment Form are grouped into five scales that represent areas of child development: Social-Emotional, Language, Cognitive, Motor Skills, and Hygiene/Self-Help.
